The SHAPE program has a dual emphasis on housing and employment supports. The goal of this program is to rapidly implement Supportive Housing and Supported Employment services to assist individuals in achieving and sustaining independent living in housing of their choice, while providing supports that also assist with obtaining or maintaining employment. The program is also designed for eviction prevention supports so people can maintain their home while actively working on their employment goals. The program utilizes principles of respect, choice, empowerment, natural supports, strengths-based, recovery, resiliency and hope.

Peer Support Housing Specialist and Peer Support Employment Specialist will meet the individuals where they are at using evidenced based practices; this includes Permanent Supportive Housing, Supported Employment and Motivational Interviewing. Individuals participating in the program choose their own housing, receive individualized supports and are treated as regular tenants.  They control their living environment. Their housing will be integrated into the community, and skills will be learned in the community where they live, work, and play. Supported Employment services will assist individuals in identifying and obtaining employment conducive to their vocational, social, psychological, and physical needs. The program actively collaborates with housing, employment, behavioral health and medical providers.
