Transitional Assistance Population
Marc Community Resources, Inc’s Transitional Assistance Population 1:1 Services are designed for persons released from the hospital setting into the community with high risk behaviors requiring additional supports for treatment. Persons may receive services within another’s providers setting or in their own home. Person’s treatment plans are reviewed weekly with Marc Center clinical team members and the RBHA. The goal is to reduce 1:1 support services and transition persons to the next step down level of care such as Level II Residential or Community Placement. Marc Center works closely with the person’s clinical team and the RBHA to ensure supports enhance the person’s recovery process.
