Behavioral Health Community Integration Services provides housing and support services for individuals with a serious mental illness in order to assist them to successfully live in a home setting by providing the supports necessary to assist them in achieving their recovery goals. Housing supports include:

COMMUNITY BASED LIVING

Marc’s Behavioral Health Independent Community Based Housing services are designed to provide support services to adults with serious mental illness. Services promote recovery and are provided to individuals in their own homes. The goal is to assist individuals as they transition into the community and achieve the greatest level of independence possible. Services are currently provided in: Gilbert, Mesa, Tempe, and Phoenix. There are house models as well as apartment settings. Staff members provide training in cooking, budgeting, health and safety, as well as help with daily living skills to foster independence and to promote recovery.

RESIDENTIAL SERVICES

Marc’s Behavioral Health Residential Treatment Facility services are designed to provide short term structured support services to adults with serious mental illness. Services promote recovery and are provided in a structured residential treatment setting with 24-hour supervision, including counseling or therapeutic activities and assistance with the self-administration of medications for people who do not require onsite medical services. The goal of Residential Services is to enhance the independence, dignity, personal choice and privacy of the individuals. Services are currently provided in Mesa.

INTENSIVE SUPPORT SERVICES

Marc’s Intensive Support Services program is designed for people discharged from the hospital setting into the community with high risk behaviors requiring additional supports for treatment. Individuals may receive services within another provider’s setting or in their own home. The individual’s treatment plans are reviewed weekly with the individual’s Adult Recovery Team members and the Regional Behavioral Health Authority (RBHA).
The goal is to reduce the level of support services and transition the individuals into a more independent environment.

Marc works closely with the individual’s clinical team and the RBHA to ensure supports enhance the individual’s recovery process.

EAST VILLAGE

The East and West Village programs are aimed at engaging individuals in meaningful community activities to promote recovery and foster resilience. The program uses the SAMHSA Illness Management and Recovery Model.

WEST VILLAGE

The East and West Village programs are aimed at engaging individuals in meaningful community activities to promote recovery and foster resilience. The program uses the SAMHSA Illness Management and Recovery Model.

HEALTH & WELLNESS PROGRAM

Health and Wellness includes certified fitness specialists and trained peer wellness coaches who perform fitness assessments and coach individuals in their personal fitness goals in order to improve their ability to perform activities of daily living and manage physical health conditions.  The program uses the Stanford Chronic Disease Self management Model.

IN-HOME SUPPORT FOR ADULTS

The In-Home Support program offers individuals in-home support services to allow the person to continue living in their home and participate in their community.

NUTRITIONIST SERVICES

Services are provided throughout the community such as: nutrition groups, meal planning, carbohydrates, protein, fat, vitamin and minerals, metabolism, nutrition trends, diet myths and much more.
